Invoice/s to be paid in full on day of assessment. The rate and or total cost of any service received will be agreed upon prior to MBB Health Ltd or Better-Health.UK undertaking and entering into any agreement or contract with (You**). Once an appointment for any service has been agreed and booked, you have 14 days from time of booking to change your mind and receive a full refund of your deposit, if cancelled by you after this period £50.00 non-refundable booking fee applies unless an appointment is rebooked within 3 calendar months of the original appointment date, a maximum of two consecutive appointments apllies after which the non refundable policy applies) . At the discretion of MBB Health/Better-Health.UK it may be refunded in exceptional circumstances. Please note, this is an assessment and advisory service not a diagnostic and treatment service. (You) agree to the gathering and recording of physiological vital signs, past and current medical history including current medication or any relevant medical/social/lifestyle information in relation to the assessment and/or service entered into.All health and personal information within the assessment document is provided by you (the client) and remains confidential, Better-Health will not accept any liability for any omissions or incorrect health information you provide, nor will it accept any liability for your health and wellbeing following an assessment or any other service entered into. If you are having follow up appointments with Better-Health.UK please keep the assessment paperwork secure for your next visit. The wellbeing assessment is measured against recognized health and wellbeing scoring criteria designed to identify potential health conditions (Eg: NICE, PHE, HEE, SIGN) it is not a definitive diagnosis of a particular medical/health condition and you are advised to seek the advice and treatment of your registered GP should you be aware of or are assessed as having a potential health condition. Disclaimer: The physiological measurements captured on the day of the assessment are relevant during the assessment and they are not indicative of what they will be in the hours, days, weeks and months following a health and wellbeing assessment. Better-Health or MBB Health Ltd does not accept liability for your health and wellbeing following any assessment. You enter into this agreement with this understanding. Do not make any changes to your medications unless directed to do so by your GP. Better-Health.UKBetter-Health.UK will review and
assess but 'we' will not tell you to change your regular medications as the assessment is a guide for you to discuss with your registered GP.
MBB Health Ltd is exempt from CQC registration under the Health & Social Care Act 2008. The Scope of Registration
(May 2022) Sections: Regulated Activities 42-45. © Care Quality Commission 2022.The health and wellbeing information gained on the day of the assessment or service entered into will not be shared with any individual, organization or company in accordance with GDPR 2018. It remains your responsibility to keep it safe and to inform your registered GP of the information gathered from any service you receive and for you to forward a copy of it onto to him/her (a £10.00 fee a copy can be provided). It is also your responsibility to follow up on any health and wellbeing advice or recommendations given in the assessment feedback or service received from Better-Health or MBB Health Ltd, MBB Health Ltd (Better-Health.UK) will only share your name and address for accountancy and HMRC purposes. We will not disclose the reason for the assessment/contract entered into and we will not disclose any personal health information, this remains confidential. It is your responsibility to keep it safe and secure. You will receive your assessment and any relevant paperwork on the day of the assessment, it will also be emailed to you should you wish.
You (the client) understand that by entering into and accepting any service offered by MBB Health Limited (Better-Health.UK) is entirely of your own volition and you have the right to withdraw that consent at any time by informing Better-Health - MBB Health Limited in writing/email. You also confirm that you have read and fully understood the information provided above and I wish to proceed with the agreed health and wellbeing assessment or other service(s) agreed and entered into with MBB Health/Better-Health.
NB: By signing up to this agreement (you/client) are entering into a legally binding contract with MBB Health Limited/Better-Health.UK and (you/client) agree to the T&C’s stated above.
You will be provided with a printed copy of these terms and conditions on the day of the assessment to sign.
